How do I access Veteran Community Nursing Services?

Accessing our community nursing services for veterans is simple. If you are a DVA-eligible veteran living in Adelaide, South Australia, you will require a written referral for Community Nursing services from one of the following:

  • general practitioner (GP)
  • Treating medical practitioner in a hospital
  • Hospital discharge planner
  • Nurse practitioner specialising in a community nursing field

What happens once a DVA Community Nursing referral is submitted?

Assessment by Registered Nurse

Upon receiving your referral, a registered nurse will assess your needs. This assessment includes confirming your eligibility with DVA (if you hold a Veteran Gold Card ot Veteran White Card) and evaluating your requirements for community nursing and personal care services.

Tailored Care Plan

Following the assessment, the nurse will prepare a personalised written nursing care plan specifically tailored to your needs. This plan outlines the care and support you will receive, ensuring it aligns with your unique circumstances and preferences.

Care Provision

Once the care plan is finalised, our team will begin providing the services outlined in the plan, delivering compassionate and professional care to support your well-being and independence at home.
